The RN Community Health role provides day‑to‑day clinical knowledge, coaching, and health screenings to vulnerable populations. They may also provide some oversight and operational coordination for Community Health Workers (CHWs) and other frontline staff delivering community‑based services. This role blends hands‑on clinical practice with relational community work, ensuring safe, high‑quality care in home, community, and outreach settings. This role supports access to health, chronic disease management, social determinants of health interventions, and care navigation.

The RN will serve as a bridge between the community and the health care, government and social service systems. Assists individuals, families, groups and communities access resources, including health insurance, food, housing, quality care and health information. Provides informal counseling, support, and follow-up. 

The RN will provide health screenings including blood pressure, diabetes, lipids and social determinants of health with care navigation. They will also provide immunizations at some community events.

This position requires travel to various community settings—including homes, schools, and community centers—as well as performing office-based duties. A valid driver’s license and reliable transportation are required to conduct community visits.  

Schedule: 8:00am-4:30pm 

**Must be able to flex up to 25% of time to cover community events taking place during nontraditional work hours (Evenings and Weekends).**

  • Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program.
  • Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
