HudsonView Center is seeking a dedicated and experienced Nurse Practitioner (NP) to join our team in Hudson County, NJ. This is an impactful role centered on providing high-quality, patient-centered care within post-acute and long-term care settings. Collaborate with a supportive interdisciplinary team to drive excellent patient outcomes.

Role Focus & Responsibilities

The primary focus is managing complex medical conditions to reduce hospital readmissions and ensure seamless patient transitions.

Clinical Care: Conduct comprehensive patient assessments, diagnose conditions, and develop and manage complex care plans.
Documentation: Accurately document all patient encounters and treatments using Electronic Health Records (EHR).
Collaboration: Work seamlessly with physicians, nursing staff, and other healthcare professionals.
Communication: Serve as a key communicator with patients’ families and support systems.
Participation: Attend facility meetings and actively contribute to care planning discussions.
Standards: Maintain the highest level of professionalism and uphold HudsonView Center's commitment to clinical excellence.

Qualifications

Required

Education: Master’s or Doctorate degree in Nursing from an accredited program.
Licensure: Active New Jersey APN and RN Licenses.
Certifications: Active DEA, CDS, and BLS certifications.
Experience: Minimum 2 years of Nurse Practitioner experience.
Expertise: Proven experience managing elderly patients and complex medical conditions.
Tech: Proficiency in electronic health records (EHR).

Preferred

Experience in post-acute care environments.
Strong procedural skills, including: IV insertion, G-tube replacements, wound care, and suture/staple removal.
