Your Responsibilities:

Develop & Implement Restorative Care Plans – Work with residents and the healthcare team to promote functional independence.
Monitor & Evaluate Resident Progress – Track the effectiveness of restorative interventions and adjust care plans as needed.
Provide Therapeutic Activities – Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.
Educate Residents & Families – Provide guidance on exercises, safety, and strategies to enhance recovery.
Collaborate with Nursing & Therapy Teams – Ensure coordinated care across all disciplines.
Document & Record Resident Progress – Maintain accurate and up-to-date records of care and progress.

 

Qualifications:

Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).
Strong Assessment & Critical Thinking Skills.
Excellent Communication & Teamwork Abilities.
A Compassionate, Patient-Centered Approach to Care.
Ability to Work Independently and as Part of a Team.
