Position Summary
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our PACE program. This hybrid role provides nursing services in both clinic and home settings, supporting participants with chronic and complex health needs. The LPN delivers direct care, supports care coordination, and helps educate participants and families. Working under the supervision of a Clinical Nurse (RN) and in close collaboration with the interdisciplinary team (IDT), the LPN ensures participants receive safe, compassionate, and coordinated care. This position also participates in the on-call rotation to meet participant needs outside of regular hours.

Essential Duties & Responsibilities

  • Provide direct participant care including medication administration, wound care, vital signs, injections, and other nursing procedures in both clinic and home environments.

  • Support care coordination by communicating participant needs, changes in condition, and follow-up actions to the Clinical Nurse and IDT.

  • Educate participants and families on disease management, medication adherence, and health promotion.

  • Assist with case management activities such as monitoring progress, ensuring care plans are followed, and reporting barriers to care.

  • Accurately document all care provided in the electronic health record (EHR).

  • Maintain safe and effective infection control practices in all care settings.

  • Promote participant independence, dignity, and safety in line with PACE principles.

  • Participate in IDT meetings, case discussions, and care planning.

  • Fulfill responsibilities as part of the on-call rotation.

  • Perform other duties as assigned to support participant care.

Essential Requirements

  • Current LPN license in the state of practice.

  • At least 1 year of experience working with the elderly population (geriatric, long-term care, home health, assisted living, or PACE preferred).

  • Current CPR/BLS certification.

  • Valid driver’s license and reliable transportation for home visits.

  • Strong communication and interpersonal skills to work effectively within an interdisciplinary team.

  • Competence in basic computer skills and ability to use an EHR system.

  • Flexibility to adapt to participant needs and program priorities.

  • Primarily weekday schedule with participation in the on-call rotation.
