Comprehensive review of the patient's medical history is performed and documented. Utilizes Teach Back technique to assess understanding of education. Assesses needs for discharge and the best way the patient may be provided instruction. Utilizes appropriate written communication, education packets and/or audio-video instructions. Assists Outpatient Infusion Manager, Professional Development Coordinator/Educators in planning and presenting unit-based in-services. Completes annual competencies for Infusion administration and Nursing. Responds to patient requests and proactively addresses patient needs using a multi and interdisciplinary approach. Works with the unit team to address patient satisfaction scores and works with unit leadership to develop and implement action plans.
