Primary Care RN or LPN - Mercy Clinics - Southfork

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards. Position Details: Mercy Clinic Southfork 12700 Southfork Road St. Louis, MO 63128 Schedule: Monday - Friday No weekends or holidays! **Must complete three months of on-site training prior to transitioning to hybrid work-from-home schedule; Must also be available to come on-site for monthly clinic meetings and 1-2 times per week** Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete. Specialty-LPN (Non-Exempt) (34003N)

Utilizes the nursing process to provide patient care under the supervision of person(s) licensed by a State regulatory board to prescribe medication and treatment, or under the direction of a Registered Professional Nurse (RN). Maintains professional accountability for provision of patient care for the assigned patients. Coordinates the patient care in conjunction with other departments. Position Details: Minimum Qualifications: Education: Graduate of an accredited practical nursing program; Consideration will be given for comparable education experience and successful licensure. Licensure: Is personally responsible for obtaining, and maintaining a current LPN license within the hiring state in which nursing duties are performed and must meet all State Board of Nursing requirements Experience: Can be new or experienced LPN Certifications: Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire NOTE: one or more of the certifications below may be required based on the position/unit hired to, or acquisition of certification within department required timeframe: • ACLS (Advanced Cardiac Life Support) • PALS (Pediatric Advanced Life Support) • PEARS (Pediatric Assessment Recognition and Stabilization) • S.T.A.B.L.E.
