Personalized Treatment Planning: Develops and implements individualized, evidence-based treatment plans incorporating pharmacologic and non-pharmacologic interventions, patient education, and interdisciplinary referrals as needed. 6. Ongoing Patient Evaluation: Monitors patient progress through follow-up assessments, adjusting treatment plans as necessary to ensure therapeutic effectiveness and alignment with patient-centered goals. 7. Scope of Practice & Collaboration: Provides care in alignment with the Nursing Practice Act, scope of service, and hospital policies, consulting with physicians and specialists as needed for comprehensive patient management. 8. Accurate & Timely Documentation: Ensures meticulous documentation of all patient encounters, assessments, interventions, and outcomes in accordance with hospital and clinic policies to maintain compliance and continuity of care. 9. Quality Improvement & Professional Engagement: Actively participates in quality assurance and performance improvement initiatives, contributing to hospital and clinic efforts to enhance patient safety, care efficiency, and clinical outcomes. 10. Resource Optimization & Cost-Effectiveness: Demonstrates responsible utilization of healthcare resources, aligning clinical practice with internal and external benchmarking, service line metrics and institutional budgetary goals while maintaining high-quality care standards. 11. Regulatory & Professional Compliance: Meets Ongoing Professional Practice Evaluation (OPPE) and Focused Professional Practice Evaluation (FPPE) standards as outlined in the Medical Staff Bylaws, adhering to all NP Standardized Procedures, policies, and protocols relevant to Eisenhower Health. 12.
