SUMMARY (BASIC PURPOSE OF THE JOB)
MINIMUM REQUIREMENTS
Education: High School Diploma or GED. Nursing assistant or home health certification. Successful completion of medication administration training course and statewide standardized, written competency test. NJ medication assistant certification.
Experience: One year of of CNA/CHHA experience.
Other Credentials: AHA Heartsaver AED. Non-Licensed must obtain before end of orientation period
Knowledge and Skills: Excellent customer service skills. Possesses a friendly demeanor. Has excellent bedside manner.
Special Training: Training in multiple age populations. Proficient in Microsoft Word.
Mental, Behavioral and Emotional Abilities: Good critical thinking skills. Possesses ability to work under stressful conditions.

ESSENTIAL FUNCTIONS
- Maintains a working relationship with the LPN, RN, DON, Assisted Living administrator and all other nursing staff, volunteers, Capital Health LIFE/Capital Health administrative staff, hospital employees, department heads and supervisors, outside vendors and contractors, caregivers, family members, and property and building staff.
- Maintains a professional relationship with all residents. Maintains confidentiality of verbal and written information pertaining to residents, ALP operations and other personnel. Maintains residents' privacy and dignity. Encourages residents to their independence.
- Possesses a full understanding of fire, emergency and safety procedures. Adheres to policies, procedures, and standards regarding infection control. Understands the hazards and safety of oxygen being used by residents.
- Checks phone messages and returns calls as needed. Reviews any new faxes, fax new orders to MDs and pharmacy. Reads communication book and daily logbooks and addresses any concerns. Checks list of residents scheduled for MD and other appointments and ensures progress notes are prepared by nurse to accompany resident. Reviews narcotics count.
- Makes sure CHAs and CNAs document in communication book properly, and that they are reading communications and following through on communications as needed. Supervises CHHAs and CNAs to ensure implementation of resident care plan and service plan.
- Assists residents with medication administration following proper medication techniques as learned in CMA course. Demonstrates the "five rights" of medication administration and safe medication practices properly.
- Maintains frequent close communication with the RN. Properly identifies medication and residents before administering medication. Documents on MARs when medication has or has not been given. Notifies LPN or RN if resident not given or refuses medication. Notifies LPN or RN immediately if medication error has occurred and document appropriately. Checks all meds daily and call pharmacy for needed refills. Administers medications including pre-filled insulin pens as per provider orders
- Monitors and documents glucose levels. Offers direct observation and supervision of the medication administration task by the RN quarterly. Receives medications from pharmacy and documents that they were delivered. Provides morning and evening personal care to assigned residents including partial or complete bath, oral hygiene, care of hair, and incontinence care including changing adult briefs as needed. Provides ostomy care.
- Takes weights and vital signs (TPR & BP) monthly and as requested by nurse. Records and reports results. Provides ambulation assistance as needed. Assists with transfer to and from bed. Reports any change in resident status to nurse immediately. Responds to emergency calls and assists nurse as needed. Reports any accidents, injuries, or complaints to nurse or administration immediately.
- Delivers supplies and equipment to residents. Provides light housekeeping. Cleans refrigerator, stove or oven, cabinets, and silverware drawers as needed. Vacuum and dust as needed.
- Assists with laundry including washing, drying, and folding laundry, and putting it away. Strips and remakes bed as needed once a week. Cleans the bathroom including bathroom floor, vanity, toilet, and shower or bathtub. Removes trash from resident apartments daily. Assists resident with meal service as needed including meal preparation and cutting up foods, set-up, clearing, and washing dishes.
- Reports any unsafe equipment to nurse/administrator. Maintains the cleanliness and neatness of the wellness office, laundry room, equipment, and inventory supplies. Removes trash from wellness offices and laundry rooms.
- Maintains continuing education requirements for medication administration.
- Abides by established guidelines of the facility and governing regulations.
- Performs responsibilities outlined in the Certified Home Health Aide job description or deemed necessary by the administrator.
- Performs other duties as assigned.
