We are currently seeking Licensed Practical Nurses (LPNs) to join our night shift team at our Skilled Nursing Facility in Conneaut, Ohio. We have 2 Full-Time and 1 Part-Time openings for compassionate, dependable nurses who are committed to high-quality resident care.

Position Details

  • Shift: Night Shift – 7:00 PM to 7:30 AM

  • Employment Type: 2 Full-Time | 1 Part-Time

LPN Responsibilities

  • Provide direct nursing care to residents in accordance with care plans

  • Administer medications and treatments as prescribed

  • Monitor residents’ condition and report changes to RN or physician

  • Accurately document care in electronic medical records

  • Collaborate with CNAs and nursing team to ensure resident safety

  • Maintain compliance with state and federal regulations

Qualifications

  • Active Ohio LPN license (required)

  • Experience in skilled nursing or long-term care preferred

  • Strong clinical and communication skills

  • Ability to work independently on night shift
