Position Summary

The Admissions Nurse (RN) oversees and coordinates the admissions process for new residents and hospital returns. This role completes clinical assessments, initiates care plans, ensures documentation accuracy, and collaborates closely with nursing leadership and interdisciplinary teams. The Admissions RN also provides supervision and clinical support as needed.

This role is ideal for an RN seeking a weekday-only schedule with leadership responsibility and strong work-life balance.

Primary Responsibilities

Admissions & Care Coordination

  • Conduct comprehensive admission assessments for new residents and hospital returns

  • Initiate and update individualized care plans

  • Review PRI assessments and manage the full admissions process

  • Participate in discharge planning and care transitions

Clinical Oversight & Documentation

  • Support nursing operations under the direction of the Director of Nursing

  • Perform audits of resident records for regulatory compliance

  • Complete required documentation, including charting and assessments

  • Prepare MDS documentation and care plans as assigned

  • Maintain HIPAA compliance and confidentiality

Leadership & Support

  • Supervise, direct, and provide resident care as needed

  • Serve as a clinical resource for interdisciplinary team members

Qualifications

  • Active, unrestricted New York State RN license

  • Strong clinical assessment and independent decision-making skills

  • Leadership and supervisory experience preferred

  • Long-term care experience preferred

  • Proficiency in Microsoft Word, Excel, and Outlook

  • Excellent communication and interpersonal skills
