Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner (PMHNP) to join our team. Responsibilities:

Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
Diagnose and develop individualized treatment plans based on the patient's mental health needs.
Provide medication management and adjust treatment plans as necessary.
Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
Offer counseling and psychotherapy services to patients and their families.
Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
Participate in clinical meetings, case conferences, and quality improvement initiatives.

Requirements:

Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
Ability to work independently and effectively manage a caseload of patients.
Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
Proficient in electronic health record (EHR) systems and basic computer skills.

The APN serves as an expert in breast health, guiding patients through diagnostic imaging, biopsies, and high- risk surveillance, while ensuring timely communication and care coordination. The APN ensures high-quality, patient-centered breast health care. Responsibilities What You’ll Do Conducts focused pre-procedure assessments, including breast history, risk factors, and relevant medical background. Performs clinical breast exams for high-risk and diagnostic patients. Assists radiologists during breast imaging procedures and biopsies, ensuring patient safety and comfort. Manages local anesthesia preparation, wound care, and patient comfort during procedures.
