Manages, coordinates and performs nursing care with compassion and care for an assigned group of patients and their families and incorporate preventative measures as directed by Registered Nurse.

Responsibilities

  • Collects patient data within the clinical standards; reports significant variances to registered nurse.
  • Establishes workload priorities and offer input into plan of care; encourages patient participation in plan of care.
  • Provides and documents care within standards of practice, policies and procedures and protocols consistent with the patient plan of care under supervisor of an RN.
  • Identifies expected outcomes individualized to patient needs under RN guidance.
  • Demonstrates competency in all required areas, including department-specific, age-specific, medication administration, and use of equipment.
  • Assumes responsibility for professional development by:
  • Maintaining ongoing continuing education,
  • Contributing to professional development of peers, and
  • Participating in CQI.
  • Performs other duties as assigned.

Qualifications

Graduate of an accredited school of practical nursing.  Current registration in Illinois for practice as a licensed practical nursing.  Six months experience preferred.

The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, administration of maintenance fluids, administration of tube feedings, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, urinary catherization, bathing, dressing, grooming, toileting, ambulating, transferring, feeding, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Immediately report any observed changes in condition or behavior of patients and documents any care given in an acceptable medical legal fashion. Immediately reports any unusual incidents. Set up for suturing and procedures, using sterile technique; dismantles procedure set, caring for soiled instruments safely and correctly. Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice. Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as, safe storage of medications. Re-order supplies as required and within budgetary limitations. Maintain work station cleanliness and restock linen and supplies to exam rooms; fills blanket warmer and IV trays; provides general cleaning, including emptying trash, straightening waiting area, etc. Washes and changes stretcher or exam table linen and empties dirty laundry.
