Responsibilities

The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician.

Essential Duties and Responsibilities

Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit
Takes history, examines, determines diagnoses.
Provides written documentation of patient visit, per NCQA standards
Takes patient vital signs, as necessary.
Places case management referrals and communicates with PCP as necessary.
Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis.
Performs all clinical duties while observing OSHA Universal Precautions
Maintains patient confidentiality
Attends required meetings and in-services and participates in committees, as requested
Participates in professional development activities and maintains professional licenses and affiliations

Qualifications

Required Knowledge, Skills, and Experience

Active/unrestricted nurse practitioner license to practice in coverage states
Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
Active BLS Certification
Current enrollment in Medicare/Medicaid
Must maintain a valid driver’s license and good driving record
Outstanding EHR skills

Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public

Requirements Is a graduate of a master’s degree in a Nursing Program. APRN certification as Family or Adult Nurse Practitioner. In-depth, current knowledge in area of specialty and the ability to translate professional and organizational goals into effective practice. Is currently licensed as an APRN through the State Board of Nursing. At least three (3) years general nursing experience is required with current hospice, home health, medical, surgical or critical care experience. Has experience with providing good pain management. I.V. skills preferred. Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Committed to the research process and application to practice. Ability to organize resources appropriately. Strong interpersonal skills, communication, collaboration and conflict resolution. Knowledge of systems and the change process. Ability to manage multiple priorities simultaneously and effectively handle the emotional stress of the workload. Complies with accepted professional standards and principles. Understands Bluegrass Care Navigators’ philosophy, principles of death/dying. Possesses and maintains current CPR certification. Have satisfactory references from nursing school, previous (or current) employers, and/or professional peers. Has excellent observation, good nursing/medical judgment and communication skills.
