Overview
We are seeking a dedicated and compassionate Family Nurse Practitioner to provide high-quality, comprehensive primary care to patients across the lifespan. This role focuses on the diagnosis and management of common and chronic conditions, with a strong emphasis on health promotion, disease prevention, patient education, and collaborative, team-based care. The Family Nurse Practitioner will work closely with the Medical Director and interdisciplinary care teams to support quality outcomes and continuous improvement in clinical practice.
Key Responsibilities

Provide comprehensive primary care services, including health assessments, physical examinations, diagnosis, and treatment planning.

Order, interpret, and evaluate diagnostic tests and develop individualized plans of care.

Prescribe medications and treatments in accordance with scope of practice and regulatory requirements.

Deliver preventive care, health screenings, counseling, and patient/family education.

Collaborate with physicians, nurses, and other healthcare professionals; make referrals to specialty and community resources as appropriate.

Manage and document patient care accurately and in a timely manner.

Provide age-appropriate care to pediatric, adolescent, adult, and geriatric patient populations.

Participate in quality improvement initiatives, committees, and special projects as assigned.

Provide telephone coverage, home or hospital visits, and on-call support as directed.

Serve as a preceptor, mentor, and role model for students and staff when applicable.

Maintain compliance with organizational policies, confidentiality standards, and regulatory requirements.
Qualifications
Education
-
Bachelor’s and Master’s degree in Nursing.

Graduate of an accredited Nurse Practitioner program.

Bilingual (Spanish preferred).
Experience & Skills
-
Strong clinical experience as a Family Nurse Practitioner.

Excellent clinical judgment, problem-solving, and leadership skills.

Effective organizational, communication, and interpersonal abilities.

Basic computer proficiency.
Licensure & Certifications
-
Current New York State Registered Nurse License and Registration.

Current Nurse Practitioner Certification and Registration.
-
Active collaborative practice agreement, as required by NYS law.
-
Certification (or eligibility) by a recognized specialty organization.

Current BCLS certification; Infection Control and Child Abuse training.
