Gurwin Jewish Nursing & Rehabilitation Center is a renowned healthcare provider located on Long Island, NY which offers a full continuum of services. The Nursing & Rehabilitation Center is a 460-bed facility providing services for short-term rehabilitation and long-term skilled nursing care, home care programs, an adult day health program, ventilator dependent care, palliative and hospice care, Memory Care Unit and on-site dialysis. The Gurwin Family of Services also includes the 201-apartment Gurwin Jewish ~ Fay J. Lindner assisted living community and the Fountaingate Gardens independent living community on the Commack campus.

We are currently seeking a per diem experienced MDS RN Nurse Assessor who will embody the passion and commitment required to deliver the ultimate care for our residents. You will be scheduled on an "as needed basis".

Responsibilities include, but are not limited to:

  • Reviews all admissions to the facility to insure that professional staff performs comprehensive assessment and completion in a timely manner.
  • Prepares/coordinates MDS schedules for quarterly, annual and significant change assessments.
  • Reviews clinical condition of residents to coordinate residents appropriate case mix.
  • Serves as liaison with all disciplines to attain the highest practical functional level for each resident.
  • Keeps abreast of new guidelines and changes within the Regulatory System. Updates and interprets guidelines as they relate to the care of residents and accurate completion and review of the MDS.
  • Communicates to all disciplines any shift comparisons made, inconsistencies noted, and conflicting information appropriately considered once the information has been assimilated.  Works with the department staff member to educate and correct information as appropriate.
  • Provides guidance on how to organize information to assist staff in thinking about problem identification, care planning and treatment decision.
  • Collects and reviews data and prepares for electronic submissions for all assessments and discharges monthly.
  • Audits the care planning process with the CAA review process to achieve a level of accuracy that represents the resident’s condition at all times.
  • Reviews all disciplines CAA notes for accuracy and quality.
  • Participates in facility readiness for State and Federal QA.

Requirements include:

  • Graduate of an accredited School of Nursing
  • Must be licensed to practice as a Registered Nurse in New York
  • Prior experience as an MDS Nurse Assessor
