The RN Clinic Supervisor provides leadership and management of the InnovAge clinic. They are responsible for the supervision of assigned staff providing guidance and monitoring the performance of the clinic staff and the direct care given to the participants. The Clinic Nurse Supervisor is familiar with a variety of the RN, LPN, MA, Outside Scheduler, Clinic Clerk, PCW and CNA practices and department procedures and relies on experience and judgment to plan and accomplish goals. The Clinic Nurse Supervisor in this position works closely with therapy staff, social services, physicians, administrative staff, patients and their families. 

 

Participant Nursing – 40%

  • Monitors compliance with all applicable licensing, certification and accreditation requirements.
  • Serves as a member on various committees both within and outside of the agency as requested by the Home Care and Clinic Service Manager.
  • Manages the flow of clinic by overseeing appointment bookings, triaging and gate keeping in conjunction with primary care and staff. Ensures that participants are cared for in a timely and appropriate fashion.
  • Observes, records, and reports participant’s condition and reaction to drugs and treatments to physicians responsible for treatment.
  • Coordinates participant care with outside contract service providers including nursing homes, assisted living, lab, oxygen and others.
  • Manages and reviews the records and charts of participants for pertinent information and staff documentation, performs documentation as necessary.
  • Encourages team building by coaching and mentoring staff as needed and training home care and clinic staff to function independently.
  • Follows up and communicates with weekend and after hour staff as appropriate.
  • Instructs participants and family regarding medications and treatment instructions.
  • Administers medications, provides training in self-administration of all non-scheduled medications and monitors medication compliance.
  • Provides health counseling to participants and their families on an individual or group basis.
  • Assists in the examination, assessment, evaluation testing and treatment of participants.
  • Perform rounds at the ADHC for the purpose of triaging and administering to the concerns of participants and Day Center staff. Documents, charts and communicates results from rounds thoroughly.
  • Ensure that exam rooms and/or triage area is stocked for necessary supplies and materials and that a list of medical supplies is maintained by appropriate staff.
  • Effectively communicates in interdisciplinary team meetings, family meetings, and clinic meetings.
    Participates within the interdisciplinary team in the formulation of Plans of Care for InnovAge PACE program participants, as well as in other interdisciplinary team settings that plan, coordinate and monitor the care of InnovAge PACE program participants

Supervision & Administration - 60%

  • Effectively supervises and manages direct reports and provides, by example and leadership, motivation and direction to employees of the organization.
  • Carries out supervisory responsibilities in accordance with the organization's policies and applicable state and federal employment laws.
  • Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; recognizing and rewarding employees; corrective disciplinary procedures and practices (under direction of supervisor and in partnership with Human Resources), addressing complaints and resolving problems.

  • Assists with performance management of clinic staff in collaboration with the Home Care and Clinic Services Manager.
  • Provides guidance to staff and team members that is consistent with both state law and company policy.
  • Orients and educates contract Clinic and Home care staff on InnovAge PACE systems and goals.
  • Solicits ideas from staff for ongoing improvement in the efficient functioning of the Clinic and Home care department.
