Overview Under the supervision of the Ambulatory Administrator or Director, Manager and Medical Director, utilizes the Nursing Process to provide care, coordination of care and patient education to a group of patients, using the medical home mode. Vaccines For Children responsibility applies in clinics that participate in the program.
