The Nurse in the Medical Telemetry unit is located on the 7th floor of St. Michael Medical Center. This unit has 32 private rooms and is a telemetry capable medical surgical unit. Our unit consists of medical patients requiring non-acute cardiac telemetry monitoring and general surgical overflow. The patient to nurse ratio is set to match staff requirements to meet the care needs of patients. The Charge RN creates shift assignments based on patient census, patient complexity, nurse experience level, and staff skill mix. All nurses are licensed in the State of Washington and some are certified in their area of practice. All nursing staff maintain competency through continuing education, department in-services, and annual updates of unit specific competencies.

ESSENTIAL FUNCTIONS

 

This job is responsible for providing a wide variety of professional nursing services which focus on the provision of quality patient care consistent with VMFH (Virginia Mason Francisan Health) standards / values, applicable regulatory requirements and scope of practice for the nursing profession. An incumbent is accountable for the quality of care provided to patients, and has the authority to direct direct care, provide education, seek resources at the unit / department level, and delegate appropriate tasks.

  • Adheres to the department’s and hospital’s policies and procedures, including safety, risk management and regulatory standards. Keeps abreast of trends, developments and regulatory requirements impacting matters within designated scope of responsibility.
  • Administers, oversees and documents the administration of medications (e.g., topically, orally, intramuscularly, subcutaneously and intravenously) as ordered by the healthcare practitioner and in accordance with hospital policy.
  • Assesses patient to include physical, psychosocial, cultural, behavioral / emotional, developmental and spiritual needs.
  • Gathers data relevant for patient care. Implements appropriate patient interventions and evaluates outcomes. Plans care appropriately based on needs identified during initial and on-going assessments. Provides holistic, comprehensive, and professional nursing care to patient within the scope of practice. Implements appropriate patient interventions and evaluates outcomes. Prepares patient for discharge and assists with coordinating post discharge needs.
  • Cultural Sensitivity and Competence: Demonstrates proper use of available communication tools / materials for effective communication and understands how the culture(s) of patient population can affect communication, collaboration and the provision of care, treatment and services. Patient Populations Served: Demonstrates knowledge and proper skills associated with the department’s defined specific populations served.
  • Demonstrates competent professional practice and engages in opportunities for professional growth. Demonstrates self accountability to meet stated deadlines for all mandatory requirements.
  • Documents care delivered and patient status utilizing the nursing process in accordance with VMFH standards of care, for example, changes in patient condition, patient responses to intervention and reassessments of pertinent findings. Receives and assures appropriateness, accuracy, timeliness and execution of physician orders.
  • Identifies opportunities for, and participates in, performance / quality improvement activities to improve clinical outcomes, healthcare delivery systems and address patient safety / satisfaction and cost containment.
  • Incorporates critical thinking into daily practice as demonstrated by quality of care provided, transfer of knowledge, follow through and patient outcomes. Leads, coordinates, and communicates the ongoing delivery of patient care services provided by members of assigned patient care team(s); considers patient acuity and the competencies of care team members in delegating and assigning work and in adjusting assignments as necessary; assesses and monitors team performance to ensure the quality of care provided is consistent with the VMFH standards and regulatory requirements.Coordinates nursing care and advocates for patient needs so that all appropriate disciplines are informed of and participating in the plan of care.
  • Prepares patient for examination / procedures. Routinely explains delays. Assesses, monitors, and intervenes as indicated during and post examination or procedure.
  • Provides health care education and guidance to patient and/or families. Educates regarding results, findings and diagnoses to patient and/or patient’s family within scope of practice.

Job Requirements

EXPERIENCE / LICENSURE

  • Graduation from an accredited school of nursing, BSN degree is preferred.
  • Minimum 6 months previous RN experience required.
  • One year of related work experience in an acute care setting is preferred.
  • Current licensure as a Registered Nurse issued by the Washington State Board of Nursing.
  • Specialty certification, as appropriate to the assigned work unit, is strongly preferred.
  • Current American Heart Association Healthcare Provider BLS certification.

PRN Night Shift RN - Substance Abuse Treatment | Pacific Sky Recovery Center | Spokane, Washington About the Job: The Registered Nurse is responsible for providing nursing care to patients. Supervising care treatment and services of patient care provided by LPNs, CNAs and other staff on assigned unit and shift. Developing, implementing, and evaluating an individual plan of care for each client. Assessment and re-assessment of patients. Communicating appropriate information with the interdisciplinary team. Reporting client concerns and changes in condition to supervisor and/or physician in a timely manner. Compensation Range: $37.50/hr-$56.00/hr Schedule: 7pm-7:30am Roles and Responsibilities: Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. Assesses client’s status on an ongoing basis and pertinent information gathered is documented. Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Ensures physician’s orders are legible if taken verbally and transcribed appropriately per policy and procedure. Accurately administers medications per ordered note and documents accurate count of controlled substance. Adjusts medications under provider’s supervision as needed; monitors lab results to validate medication compliance. Notifies physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc. Supervises self-administration of medications using safety initiative of using two means of client identification: documents and reports effects of scheduled and PRN medications. Maintains accountability of administration, recording, control and security of medications, including narcotics. Completes admission and discharge processes efficiently and accurately as directed. Confers regularly with treatment team (e.g., nurses, therapists, program managers, physician) regarding client care. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. Monitors client acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines. Assures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Gives thorough report to oncoming staff, supervisors, and physician. Provides coverage in emergency services as needed. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Graduation from an approved/accredited school of nursing. One or more years’ nursing experience within clinical area of service provided by facility preferred. Experience in behavioral health/psychiatric/addiction recovery environment preferred. LICENSES/DESIGNATIONS/CERTIFICATIONS: Valid current RN license from the state or from a compact state, where applicable. Certified in Psychiatric Nursing preferred.
