This innovative model of care delivery is designed to enhance the community’s access to outpatient services and improve coordination with the Hospital’s inpatient programs.

The Broadway Practice has transformed how we provide care to the Washington Heights community through its successful pilot of the Interdisciplinary Care Team, core to the Patient-Centered Medical Home model. All members of the care team—Nurses, Nutritionist, Care Manager, Community Health Worker, Patient Financial Assistants, Social Worker, Medical Assistants, and Physicians collaborate to improve the health and well-being of our patients.

Preferred Criteria:
•   Minimum three years of experience as a Nurse Practitioner in Internal Medicine
