Your Responsibilities:

· As the MDS Lead, you’ll play a crucial role in coordinating and overseeing the assessment and care planning process. You’ll ensure compliance with federal regulations, optimize reimbursement, and collaborate across departments to provide high-quality resident outcomes.

· Manage the RAI process, including timely and accurate completion of all MDS assessments

· Lead interdisciplinary care plan meetings

· Educate and support clinical team members in MDS procedures and documentation

· Monitor quality measures and support improvement initiatives

What We’re Looking For:
MDS experience is a MUST – minimum 2 years preferred
Active RN license in PA
Strong knowledge of Medicare/Medicaid reimbursement and RAI guidelines

The Home Health Weekend Registered Nurse is a field employee who: Performs skilled nursing visits and completes coordination of client care, Point of contact for all disciplines involved with providing care to patients Oversees the frequency of visits for the episode.
