ALAY Home Care is seeking a  Licensed Practical Nurse!

 

We need Caregivers who are enthusiastic, motivated, dedicated and passionate about caring for others!

 

Pay Rate: $35 per hour

LPN Job Responsibilities:

  • Plans and manages patient care according to each patient’s needs.

  • Obtains patient vital signs, including pulse, blood pressure, temperature, and respiration.

  • Provides routine care for patients.

  • Observes patients’ health.

  • Reviews medical charts.

  • Communicates with patients and their families.

  • Knows how to administer a G tube medication and nutrition

  • Provides wound care.

  • Prepares and administers injections and/or enemas.

  • Assists with patient comfort.

 

Requirements:

  • High School Diploma, Equivalent or Higher

  • Valid CPR/First Aid Certification

  • NJ LPN License

 

 

Training and Pre-screening:

  • Criminal Background Check - Fingerprints

  • Drug screening

  • College of Direct Support - Online classes for DDD Certifications

 

Preferred

  • Vehicle for Community Transport

  • 1 year Personal Care Experience

  • 1 year Clinical Experience as a Licensed Practical Nurse (LPN)

Professional nursing practices, requiring specialized knowledge, judgment, and skill are performed in accordance Bayhealth policies. After competency verification, the Licensed Practical Nurse may deliver patient care services to the adult, and geriatric populations. The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, administration of maintenance fluids, administration of tube feedings, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, urinary catherization, bathing, dressing, grooming, toileting, ambulating, transferring, feeding, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities. Collects and records intake and output. Prepare and administer oral medications as ordered by the physician or APC. Immediately report any observed changes in condition or behavior of patients and documents any care given in an acceptable medical legal fashion. Immediately reports any unusual incidents. Set up for suturing and procedures, using sterile technique; dismantles procedure set, caring for soiled instruments safely and correctly. Maintain patient confidentiality; comply with HIPAA and compliance guidelines established for the practice. Ensure compliance with Joint Commission regulations and other safety requirements; this includes disposal of used sharps and contaminated materials, as well as, safe storage of medications. Re-order supplies as required and within budgetary limitations. Maintain work station cleanliness and restock linen and supplies to exam rooms; fills blanket warmer and IV trays; provides general cleaning, including emptying trash, straightening waiting area, etc. Washes and changes stretcher or exam table linen and empties dirty laundry. Demonstrate the ability to reflect knowledge of current professional practice standards, laws and regulations, evaluate their own nursing practice in relation to professional practice standards, relevant statutes and regulations.
