Job Summary:

A leading rehabilitation and nursing facility is seeking a dedicated and compassionate Case Management Registered Nurse/Licensed Practical Nurse (RN/LPN) to join our team. Registered Nurse/Licensed Practical Nurse (RNAC/LPNAC) Responsibilities:

Assess patients' physical, emotional, and psychological needs to develop and implement individualized care plans
Coordinate patient care activities, including medication management, wound care, and rehabilitation therapies
Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure seamless patient care
Conduct patient education and discharge planning to promote successful transition to home or next level of care
Maintain accurate and detailed records of patient care, progress, and outcomes
Participate in quality improvement initiatives to enhance patient care and satisfaction

Registered Nurse/Licensed Practical Nurse (RNAC/LPNAC) Requirements:

Current Pennsylvania RN or LPN licensure
Prior experience, preferably in a skilled nursing facility
Ability to work full-time hours (40 hours/week)
