Registered Nurse/Licensed Practical Nurse (RNAC/LPNAC) Responsibilities:

  • Assess patients' physical, emotional, and psychological needs to develop and implement individualized care plans

  • Coordinate patient care activities, including medication management, wound care, and rehabilitation therapies

  • Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure seamless patient care

  • Conduct patient education and discharge planning to promote successful transition to home or next level of care

  • Maintain accurate and detailed records of patient care, progress, and outcomes

  • Participate in quality improvement initiatives to enhance patient care and satisfaction

Registered Nurse/Licensed Practical Nurse (RNAC/LPNAC) Requirements:

  • Current Pennsylvania RN or LPN licensure

  • Prior experience, preferably in a skilled nursing facility

  • Ability to work full-time hours (40 hours/week)
