QUALIFICATIONS: Current Home Health Aide Certificate in the State of New York RESPONSIBILITIES: Assist with patients Personal Care needs: personal hygiene, bathing, dressing/undressing, and ambulation together with any household task (eg making beds, changing linen, laundry, light housekeeping, etc) essential to the patients’ health in accordance with the patient's individualized Plan of Care. Plan/Prepared nutritious meals and assist in feeding patients as directed. Document services provided in accurate and timely manner in accordance with company policy and personal care plan. HHA Duties & Responsibilities: Assisting residents with activities of daily living, such as bathing, dressing, grooming, and toileting Documenting care provided and reporting any changes in resident health or behavior to appropriate staff.
