Overview:

Coordinate and manage care plans for long-term care residents
Serve as a liaison between residents, families, and healthcare providers
Ensure compliance with all regulatory and company standards
Monitor resident outcomes and support care teams in achieving goals
Provide guidance, education, and advocacy for residents and families

Requirements:

Active RN or LPN license in the state of Ohio in good standing
Minimum 3 years experience in long-term care, skilled nursing, or case management required
Strong communication, organizational, and problem-solving skills
Comfort working independently in a remote environment
Compassionate, resident-centered mindset

Hours of Job Two Day Scheduler-Scheduled to work EVERY weekend for 48 hours per pay and will earn 72 hours per pay. Duties and Key Responsibilities Responsible for delivery of patient care to the antepartum, intrapartum, postpartum, newborn, and gynecologic patient populations utilizing the nursing process of assessment, planning, implementation, and evaluation. Directs and guides patient teaching and activities of Nursing Ancillary Personnel. Assumes a lead role in patient teaching. Places the patient at the center of care practices.
