Facility: Townhouse Center for Rehabilitation & Nursing Location: Uniondale, New York Position: Home Health Aide (HHA) Shift: PRN (as needed) Pay Rate: $17/hour Job Summary: Townhouse Center for Rehabilitation & Nursing is seeking a compassionate and reliable Home Health Aide (HHA) to join our team on a PRN basis. The ideal candidate will assist residents with transportation to medical appointments, provide support during outings, and ensure safety and comfort at all times. Key Responsibilities: Escort residents to and from clinic appointments and dialysis sessions. Monitor and prevent falls or altercations, intervening promptly when needed. Assist residents in preparing for appointments and ensure snacks are provided during out-on-pass periods. Report all incidents and accidents promptly to the Clinical Coordinator or ADNS. Represent the facility professionally, adhering to all policies and procedures both on and off the property. Minimum Qualifications: Current New York State Home Health Aide (HHA) certification.
