Position Summary:
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse or Physician. The LPN is responsible for delivering safe, compassionate, and effective nursing care while supporting residents’ well-being and maintaining compliance with regulatory standards.
Key Responsibilities:
Provide direct nursing care to residents according to care plans and physicians’ orders.
Administer medications and treatments accurately and in a timely manner.
Monitor and document residents’ health status, reporting any significant changes to the RN or physician.
Assist with resident assessments and the development of individualized care plans.
Supervise and direct CNAs and other nursing assistants in delivering daily care.
Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations.
Respond to emergencies and follow proper protocols.
Support residents and families with health education and emotional support.
Maintain compliance with infection control, safety, and HIPAA standards.
Participate in interdisciplinary team meetings, audits, and in-service training.
Qualifications:
Valid LPN license in the state of employment
Graduate of an accredited Licensed Practical Nursing program
CPR/BLS certification (required or able to obtain upon hire)
At least 1 year of nursing experience (long-term care preferred, but not required)
Ability to communicate effectively and compassionately with residents, families, and team members
Familiarity with electronic medical records (EMR) systems preferred
Strong critical thinking, organizational, and time-management skills
