  • Conducts patient assessments and determines the health status, level of care, as well as any subsequent changes.
  • Ensures that the RAI completion is done accurately and timely.
  • Coordinates interdisciplinary participation in completion of the RAI and resident care plan.
  • Assists with development and implementing programs and procedures to maximize the reimbursement potential for the facility and improve quality of care.
  • Analyzes all QI/QM data with the Director of Nursing to identify trends on a monthly basis and serves on, participates in, and attends various committees of the facility.
  • Facilitates the Care Management Process by engaging the resident and all other appropriate staff members.
  • Ensures timely submission of all Minimum Data Sets to the state data base and ensures that all necessary follow-up action is taken.
  • Actively communicates with the Administrator to depict regulatory risk and clinical trends that can impact resident care as well any additional information that can have a potential effect on the clinical and operational outcomes of the facility
  • Acts as a team player with the facility’s staff to choose the correct assessment of residents and further acts as an in-house Case Manager.
