Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner to join our team. Responsibilities:

  • Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes.
  • Diagnose and develop individualized treatment plans based on the patient's mental health needs.
  • Provide medication management and adjust treatment plans as necessary.
  • Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care.
  • Offer counseling and psychotherapy services to patients and their families.
  • Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
  • Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards.
  • Participate in clinical meetings, case conferences, and quality improvement initiatives.

Requirements:

  • Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP).
  • Ability to work independently and effectively manage a caseload of patients.
  • Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities.
  • Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
  • Proficient in electronic health record (EHR) systems and basic computer skills.
  • Compassionate, patient-centered approach to care.
