Job Description

The nurse will coordinate and lead the patient care team to develop, implement and evaluate the plan of care in accordance with the Illinois Nurse Practice Act.

6:00pm - 6:00am - PRN

Pay based on experience starting at $22.72 - LPN and $29.08 - RN

Responsibilities

Administers medications and treatments according to protocol.  , A plan of care is initiated at admission with focus on patient’s identified physiological, psychological, and environmental factors.  
Identifies patient problems on integrated flowsheets and documents against the plan of care.  , Coordinates discharge planning with all team members. , Demonstrates the ability to handle emergency situations in a prompt, precise and professional manner.  , Documents accurate and ongoing assessments of patient status every 2 hours or as needed.
Documents patient teaching/education about plan of care., Initiate appropriate pathways of inpatient physician orders., Monitors that each caregiver is productive and serves as a source to provide and/or recommend education if needed., Notifies team members that an assessment is required as per physician order. , Performs patient assessments each shift of work and documents at time of assessment.  , Provides and coordinates patient and family education and documents appropriately.
