LPN Hospice Full-time

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice is looking to add a dedicated and compassionate LPN / Licensed Practical Nurse to maintain the comfort and optimize the quality of life of terminally ill patients and their families under the supervision of the Registered Nurse.

Responsibilities:

  • Provide direct care at the patient’s place of residence as indicated.
  • Work collaboratively as a member of the Interdisciplinary Team to assure that the aims of continuity and comprehensiveness of care are accomplished.
  • Understand Hospice philosophy, principles of death / dying.
  • Compassionate communication with their patients and their families.
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Collaborates with the patient and family to promote the physical well-being and address coping issues that may arise.
  • Prepare documentation for the clinical record per Hospice standards in a timely manner.
  • Participate in taking on-call responsibilities as necessary including weekends and evenings.
  • Adhere to practice of confidentiality regarding patients, families, staff and the organization.
  • Have observation skills and knowledge of when to alert doctors and other staff about changes in the patient’s condition.
  • Reporting patient’s status regularly to RN Case Manager.
  • Oversee Serenity Hospice Home Health Aides in the Facility.
  • Assesses for infection control for protection of the patient as well as family/caregiver and staff safety, as well as reporting any suspicious infection to the infection control nurse and or hospice supervisors.
  • Will be committed to ensuring that Serenity Hospice Care provides care and services of the highest quality and safety, and will continually strive to improve performance.

Requirements:

  • Licensed Practical Nurse in the state of New Jersey.
  • Hospice experience preferred.
  • Strong communication and interpersonal skills required.
  • Computer proficiency.
  • Current driver’s license and auto insurance.
  • Unlimited access to a reliable car.
