Professional Staff Nurse - Home Health Beaver County

UPMC Home Health is hiring a full-time Registered Nurse (RN) for the Beaver, Rochester, Freedom and surrounding areas.

Schedule/Shift: Monday – Friday 8am-4:30pm with rotations for on-call, weekends, and holidays.

Responsibilities:

  • Coordinate case management activities within an interdisciplinary healthcare team to ensure timely and appropriate delivery of patient services.
  • Conduct initial and comprehensive patient assessments to inform individualized care planning.
  • Establish and implement patient care plans in collaboration with physicians and other team members, aligning with physician orders and patient needs.
  • Provide direct patient care, monitor outcomes, and adjust nursing interventions to optimize results.
  • Facilitate ongoing communication with physicians, patients, families, and interdisciplinary team members, including health education and support.
  • Assign and supervise appropriate team members, including Licensed practical Nurses (LPNs) and Home Health Aides, to support care delivery.
  • Participate in case conferences, team meetings, and other collaborative forums to support coordinated care.
  • Maintain accurate and timely documentation of assessments, treatment plans, and physician orders using the agency's electronic health record system.
  • Ensure same-day data transmission in compliance with agency and regulatory standards.
  • Attend compliance training and adhere to organizational policies, procedures, and standards of conduct.
  • Demonstrate a commitment to customer service and strive to enhance patient satisfaction through professional and compassionate care.
  • Accept and apply constructive feedback to improve clinical knowledge, skills, and performance.
  • Exercise independent judgement in addressing both emergent and routine patient care needs and escalate concerns appropriately.
  • Identify and contribute to process improvement initiatives that enhance service quality and operational efficiency.
  • Participate in weekend and on-call coverage rotations in accordance with agency guidelines.
  • Exhibit proficiency in delivering care aligned with UPMC's standards, including complex and specialized patient needs.
  • Recognize personal limitations and seek guidance to meet performance goals and uphold quality standards.
  • Zero (0) to two (2) years of experience preferred
  • Six (6) months of nursing experience in an Acute Care or Skilled Care Facility or Home Health or Hospice Care preferred
  • BSN preferred

Aids clinical staff in follow up communication with MD offices and referral sources as applicable. Coordinates all incoming orders associated with census to ensure appropriateness and accuracy and timeliness of care, enters into Medical Record, and coordinates with scheduling as appropriate. Assists in document control processes, including but not limited to, review of Face-to-Face documentation and physician orders for home health. Reports problems immediately to the Clinical Manager. Triages and directors all incoming communications during workday hours for the current census assigned and follows up to facilitate appropriate service delivery. Current Registered Nurse License Required One-year recent home health experience preferred. Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify appropriate resource for hand off. Must have requisite knowledge of current Home Health Medicare, Medicaid, State and Federal regulations and Joint Commission accreditation standards. Must be able to demonstrate problem-solving abilities and have excellent telephone, interpersonal and written communication skills. Strong commitment to teamwork and customer service, both internally and externally, is required. Must have strong/proven data entry and general computer skills. Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty. PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP).
