Job Description:

The RN/LPN participates in the day-to-day coordination and delivery of skilled nursing services and performs related divisional teaching/visit duties to ensure the highest quality of care.

Duties and Responsibilities:

Performs client visits as needed with authorization by the Division of Senior and Disability Services, as well as reassessment visits for In-Home Services and Advanced Personal Care (APC) clients.
Weekly Medication set up
Monthly Diabetic Nail care
General Health Evaluation
Skin Assessments
Nursing Assessments

Qualifications, Knowledge, Skills, and Abilities:

Graduate of approved School of Professional Nursing.
Current state Registered Nurse or Licensed Practical Nurse license.
Minimum one (1) year of experience in professional nursing within the last three (3) years. Prefer a minimum of six (6) months of experience with Medicaid In-Home Services nursing.
Geriatric nursing experience helpful.

Job Summary: The LPN provides skilled nursing services to patients in their homes to promote the patient’s plan of care including to assess, plan, implement, document, and evaluate care to patients and families. The LPN supplements the nursing care needs of that patient as provided by the RN. These services are performed in accordance with the physicians’ orders and in collaboration with Haven Home Health RNs, therapists, and other clinicians.
