As a registered nurse care coordinator (RNCC) at UAB Medicine, you'll join a Magnet-designated nursing team to provide the highest quality of care, even for the most complex cases. UAB Medicine serves a diverse patient population, allowing RNCCs the opportunity to develop and expand their skills. The UAB Care Transitions Department includes more than 200 registered nurse care coordinators and social workers who are integral members of the interdisciplinary team. RNCCs provide care in both the inpatient and ambulatory settings, including the Emergency Department, with a focus on screening and assessing patients for any post-discharge needs, providing referrals for in-home care, and participating in care coordination and discharge planning for patients across the continuum of care. Job Description
To coordinate, negotiate and obtain services and resources for the care of patients. To review case criteria to determine medical necessity for admission and continued stay. To provide discharge planning and care coordination that identifies the most cost effective methods of patient care.
Most positions require 4 weeks of orientation, minimally, up to 7 weeks, based on previous experience. Orientation is completed Monday-Friday with a dedicated preceptor.
Qualifications
Bachelor's degree in Nursing and three (3) years of related experience required OR Associate's degree in Nursing and five (5) years of related experience required. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) from the American Heart Association (AHA) may be required.
