Job Description To manage the plan of care for a caseload of patients, assuming 24-hour responsibility for patient caseload. To serve as the liaison with the multidisciplinary care team. To plan care management as achieved through coordination of care with physicians, nursing staff and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources. May coordinate patient follow-up clinics and schedule tests and surgeries. Qualifications Bachelor's degree in Nursing and three (3) years of relevant clinical nursing experience OR Associate's degree in Nursing and five (5) years of relevant clinical nursing experience required. Nurse Practitioner or Physician Assistant - Weight Loss Medicine

Position Description Advanced Practice Provider - UAB Weight Loss Medicine (Part-time, benefited) The following APPs may be considered for this role: Family Nurse Practitioner, Adult Primary Nurse Practitioner, or Physician Assistant. Please note: Adult Acute Care NPs may be considered as well, but Family NP, Adult Primary and Physician Assistants are strongly preferred for this role. Work Schedule Part-time, benefited (20 hours per week) Day shift schedule with some telehealth. Telehealth sessions are TBA based on clinic & APP availability. CME allowance: Part-time benefited and Full-time benefited APPs receive an annual $2,000.00 CME allowance. This can be used for recertification, fees, CMEs, etc. that occur after the selected candidate's hire date. Position Summary: To provide medical care and treatment within established scope of practice to medical services patients as assigned by and under the direction of the collaborating physician (Section 610-X-5 of the Alabama Board of Nursing Administrative Code) or under supervision of a Board-Certified Physician (Section 540-X-7 of the Alabama Board of Medical Examiners Administrative Code). The collaborating physician shall be available for direct communication for consultation and referral or arrange for the same medical coverage by a physician who is pre-approved by the State Board of Medical Examiners in the same manner as the collaborating physician of record. To coordinate plan of care, discharge and specialty referrals as appropriate to meet the age specific needs of the patient. To plan and coordinate health care education for patients' family and other health care providers as needed. To perform comprehensive physical examinations and assessments. To interpret, analyze and evaluate patient’s medical record and social data. To perform and/or assist the physician with procedures per collaborative agreement. To coordinate and participate in performance improvement activities. Key Duties & Responsibilities Based on thorough physical and psychosocial patient assessments, formulates and implements comprehensive patient care plans focused on prevention, diagnosis and comprehensive treatment of disease. Plans include patient and family education, prescriptions, additional lab and diagnostic testing, and therapies. Ensures complete documentation of patient assessments, patient care plans, and care provided. Orders and interprets laboratory and other diagnostic tests for the patient. Maintains records and reports as required by regulatory agencies and The Joint Commission (TJC). May coordinate and/or participate in performance and quality improvement activities. May assist in the coordination of research protocols, serving as a patient advocate and a liaison between the patient and other health care providers. Provides assistance in orientation and/or training of new clinical personnel as needed. Provides educational information by conducting workshops, in-service training and distributing appropriate materials for patients and staff. Serves as the expert clinical resource to the clinical staff and patients in the area of specialty. Provides a safe environment for patients and others. Demonstrates a responsible attitude toward continued professional growth. Performs other duties as assigned. Position Requirements Minimum Requirements: Master's degree in Nursing or graduate from an accredited Physician Assistant program required.
