Responsibilities:

  • Provide high-quality, compassionate care to clients in their homes, including assistance with daily living activities, personal care, and medication management

  • Develop and maintain strong relationships with clients, families, and healthcare professionals to ensure continuity of care

  • Collaborate with the interdisciplinary care team to develop and implement personalized care plans

  • Maintain accurate and timely records of client care and services provided

  • Demonstrate a commitment to delivering exceptional customer service and promoting a positive, supportive environment

Requirements:

  • 1 year of experience in the healthcare industry or related field

  • High school diploma or equivalent required

  • Ability to lift up to 25 pounds and perform tasks that require bending, stooping, and standing for extended periods

  • Valid driver's license and reliable transportation (for travel between client homes and other destinations)

Provides care to residents or passes medication per the direction of the Care Team Manager/ALM/BHM or Executive Director. Qualifications High school diploma or equivalent required. Caregiver Med Passer and/or Caregiver Lead experience required. Experience in a healthcare environment. Staff scheduling or similar experience a plus. Strong organizational skills and the ability to work independently and adapt to changing situations as daily labor management changes. Ability to organize and maintain accurate electronic and paper filing systems. Knowledge of personal computers and related applications. Ability to identify and efficiently solve problems in a timely manner. Must be reliable, dependable and display a professional disposition.
