Primary Care RN or LPN - Mercy Family Medicine - Pacific

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards. Position Details: Primary Care RN or LPN - Mercy Family Medicine - Pacific Schedule: Monday - Friday No weekends or holidays! **Must complete three months of on-site training prior to transitioning to hybrid work-from-home schedule; Must also be available to come on-site for monthly clinic meetings and 1-2 times per week** Our Primary Care Nurse is a newly created role for the clinics. In this position the nurse will work closely with the Providers and manage triage calls/messages for the clinic. They will have the option to move hybrid after training is complete. Overview: Prepares patient for visit with provider by completing Advanced Standard room process including entering the chief complaint; taking patient vital signs; reviewing patient history and allergies; completing a thorough medication reconciliation; working the Encounter Guide including helping to close any open care gaps such as fall risk, depression screening, breast cancer screening, colorectal cancer screening, and diabetic eye exams; rechecking elevated blood pressures; and completing needed documentation in Epic for yearly preventative visits as needed. Assists with any needed in-office testing including, but not limited to, drawing blood, completing mental status exams, EKG’s, walking oxygen studies and point of care testing. Administer vaccines and other injections within scope of practice. Schedules follow up appointments when needed per office policy. Collaborates with providers and clinical care team to monitor and coordinate delivery of the care plan. Medical Assistant Qualifications: Medical Assistant: Education: High School Diploma or GED. Certification: Must obtain BLS certification within 90 days of hire . Preferred Experience: 6 months of clinical healthcare experience, preferably in a primary care office. Certified Medical Assistant: Education: High School Diploma or GED. Certification : Must have active Medical Assistant certification/registration (RMA, CCMA, CMA) or EMT/Paramedic is also accepted. Must obtain BLS certification within 90 days of hire. LPN Qualifications: Education: Graduate of an accrediated Practical Nursing program.
