Unit:

IU Health Bedford is a Critical Access hospital serving Lawrence and surrounding counties. We are proud to be recognized by the ANCC as a Pathway to Excellence facility that is committed to creating a positive practice environment that empowers and engages our team. 

Our Medical Surgical unit contains 25 Semi-Private Beds, and cares for a variety of conditions for all adult patients. They administer medications, monitor vital signs, change dressings, and educate patients about their procedures. They also coordinate patient care plans with other healthcare professionals and evaluate medical information.

Responsibilities:

The RN for this role helps with a variety of tasks that include, but are not limited to patient education, tele monitoring, charting, assisting patients with their Individual Treatment Plan (ITP), checking vitals, resource management, and more.

Although previous cardiac experience is preferred, it is not required. We are looking for a nurse who can connect with patients, and help provide a positive/exciting atmosphere.

Qualifications 

  • Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred.
  • Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
  • Requires that the RN has graduated from a nationally accredited nursing program.
  • Requires Basic Life Support certification through the AHA.
  • Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies.

Overview The Case Manager plans and coordinates care for patients from pre-admission through post-discharge by working collaboratively with the multidisciplinary team. Ensures that patients are assigned the appropriate level of care, receive the necessary services, and transition smoothly to the appropriate post-discharge setting. Performs accurate, timely, and effective discharge planning. Works with third-party payers to ensure coverage and administer education and prevention programs. ________________________________________ Position Overview: The RN-Case Manager for Primary Care plays a critical role in ensuring seamless patient transitions across care settings. This position involves assessment, care coordination, and communication with patients, families, providers, and payers. The nurse assists with a smooth transition from hospital discharge to care at home and facilitates patient education and safety.
