Hospice RN Case Manager 

Location: Cynthiana, KY (services Nicholas and Bourbon County)

Schedule: Monday-Friday 8am-5pm, with occasional on-call  


As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. Our service allows our team members to focus on one patient at a time providing care where the patient calls home. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role:

The Hospice RN Case Manager delivers compassionate and comprehensive health care services to Hospice patients in a home care setting, or wherever the patient calls home. Home care settings may consist of a private residence, skilled nursing facility, personal care home, and/or other interim living arrangements. The Hospice RN Case Manager’s goal for excellent patient care standards begins with active listening to create and implement a holistic plan of care based on the patient’s goals and preferences. The Hospice RN Case Manager serves as the patient advocate to support, guide, and coordinate care for patients, families, and caregivers as they navigate Hospice Care. The plan of care will include managing symptoms through pharmacologic and non-pharmacologic interventions for optimal comfort, functionality, and safety. Frequent monitoring and follow-up are required to evaluate the efficacy of treatments, plans of care, and to offer emotional support to both the patient and the caregiver. A Registered Nurse in this position will have strong communication skills, the competence to work autonomously, professionalism to work within an interdisciplinary team, and the ability to provide care with empathy. Requirements

  • A graduate of an accredited school of registered nursing.
  • Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or participating COMPACT state Board of Nursing. 
  • One year of general nursing experience is preferred. New nursing graduates will be considered.
  • Skilled in nursing practice and able to maintain professionalism in moments of emotional stress.
  • Provide care in a compassionate nature while serving a diverse population respectfully and professionally.
  • Complies with accepted professional and ethical standards and principles.
  • Understands and supports the hospice philosophy and principles of death and dying.
  • Possesses and maintains current CPR certification.
  • Have satisfactory references from nursing school, previous (or current) employers and/or professional peers.
  • Have excellent observation, good nursing judgment and communication skills. Ability to provide accurate and timely documentation.
  • Self-directed to work with minimal supervision and within an interdisciplinary team
  • Flexible and cooperative in fulfilling role obligation to meet the needs of patients and BCN.
  • Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.

PURPOSE AND SCOPE: The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP).
