The Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) provides comprehensive primary and preventive medical care to geriatric patients in community-based settings, including patient homes and nursing facilities. Care is delivered in collaboration with a CHMG physician, with a focus on optimizing health, independence, and quality of life for older adults. This role requires travel to patient residences and long-term care facilities.

Education
Master’s degree in Advanced Practice Nursing or completion of Physician Assistant program.
Certification, Registration & Licensure
NH APRN or Physician Assistant license
Current DEA license
National certification in specialty area preferred
Experience
Minimum of 2–3 years of clinical experience in geriatrics or primary care with older adults
Experience in home visits or nursing facility care strongly preferred
New graduates will not be considered for this role
Responsibilities
Obtain detailed patient histories, perform physical examinations, and develop appropriate differential diagnoses for geriatric patients
Order, interpret, and follow up on diagnostic tests
Diagnose and manage acute and chronic conditions within the scope of practice
Prescribe and manage medications according to clinical guidelines and practice standards
Provide individualized patient education focused on preventive care, chronic disease management, and overall wellness for older adults
Maintain thorough, timely documentation of patient encounters in the medical record
Collaborate closely with CHMG physicians, nursing staff, and other care team members to ensure continuity of care
