What you will do:
  • Provide physician orders per agency and regulatory requirements and within the limits of practice. The plan of care and re-evaluation occurs no less often than every 60 days.
  • Comprehensive documentation appropriately reflects patient care from admission to discharge utilizing all Oasis assessment tools. 
  • Serves as a case manager directing multidisciplinary patients' plans of care
  • Participates in the agency’s quality assurance program
  • Provides written documentation of assessment and care delivery in a professional manner according to policy and within the designated period.
Ideal Candidate:
  • Active NYS Registered Nurse Licensure/Registration
  • Active NYS Driver’ License, CPR/BLS Card, and COVID vaccinated

Duties Description Position Description The Internal Treatment Coordinator (ITC) coordinates all services, therapies and/or treatment provided to an individual by the Article 16 clinic and/or IPSIDD services. The treatment coordinator shall function as the primary point person and liaison at the clinic for individuals, referral sources, collaterals, and care managers/coordinators assigned to the person. The ITC ensures compliance with regulations which impact revenue generation and ensures there are no duplication of clinical services. PRIMARY DUTIES AND RESPONSIBILITIES: Ensure clinic intake and admission procedures are followed and associated documentation is completed. Gather relevant documentation and/or records of clinical diagnoses, personal demographics, and insurance information. Ensure documentation and information gathered is available in the Electronic Health Record. Obtain and review pertinent information from referral sources and outside service providers, ensuring there is no duplication of services. Completion of service requests in the Electronic Health Record, to be sent to the medical director/designee for review and approval. Conduct clinical review of discipline-specific treatment plans received from service providers, to ensure completeness, accuracy and compliance with all regulatory requirements. Treatment coordinator will submit treatment plan(s) that meet the regulatory driven content criterion to the medical director/designee for final authorization. Ensure written service recommendations are distributed to the individual and their collaterals including evaluation results, when a treatment plan is developed, when there are significant changes made to the treatment plan, and when the treatment plan is discontinued. Ensure the individual is receiving the services as described in both service request(s) and treatment plan(s) through the following means: review of completed evaluations, a sampling of progress notes and direct communication with service providers. Track and notify medical director/designee when annual physician reassessments are due, providing additional support as requested.
