Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our healthcare team at Woodbine Rehabilitation and Healthcare Center in Alexandria, Virginia. As a valued member of our team, you will play a crucial role in providing high-quality patient care and ensuring the overall well-being of our residents.

Responsibilities:

Provide direct patient care under the supervision of a licensed nurse
Administer medications, vaccines, and treatments as prescribed by physicians
Monitor and report patient condition to the healthcare team, including changes in vital signs and overall health
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct routine assessments and update patient records accordingly
Participate in patient education and counseling, as well as educate families and caregivers on patient care techniques
Collaborate with other healthcare professionals to ensure seamless patient care

Requirements:

Current LPN license in the state of Virginia
Minimum 1 years of experience in a long-term care or rehabilitation setting (new graduates welcome to apply)
Basic Life Support (BLS) certification
Ability to lift, transfer, and reposition patients, as well as push heavy machinery
