We’re looking for compassionate, skilled, and dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide high-quality care to our residents. Job Summary: As an RN/LPN in our Memory Care Unit, you'll provide skilled nursing care under the direction of the attending physician and within the scope of state nursing practice guidelines. You will play a key role in ensuring each resident receives individualized, compassionate care.

Responsibilities of Registered Nurses (RN) / Licensed Practical Nurses (LPN):

Monitor and assess residents for changes in condition
Administer medications and treatments per provider orders
Provide support with ADLs and personal care
Oversee and support CNAs
Maintain accurate documentation and charting
Collaborate with interdisciplinary teams to create care plans
Provide trauma-informed and culturally sensitive care
Participate in admissions, discharges, and care plan reviews
Maintain compliance with infection control, safety, and facility policies

Qualifications for Registered Nurses (RN) / Licensed Practical Nurses (LPN):

Valid RN or LPN license in the state of Illinois
CPR certification required
Compassion, patience, and a strong work ethic
Ability to work independently and as part of a team

How you make a difference The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation.
