Schedule: Thursday- Monday, from 10am-6pm, 8hrs/daily.
Under the direction of the Director of Nursing, the Charge Nurse is responsible for the oversight and
evaluation of the implementation of facility’s resident care policies and procedures affecting the
quality of patient/resident care outcomes, identifying regulatory noncompliance, and ethical business
conduct.
QUALIFICATIONS
-
A graduate from an accredited school of professional nursing.
-
Current nursing licensure in the state he or she is responsible.
-
5-year Experience in SNF.
-
Trained in nursing service, administration, rehabilitation, psychiatric or geriatric nursing.

Two or more years of demonstrated ability in the implementation of resident care Policies
-
and Procedures and/or supervision in a health facility.
-
A desire to work with geriatric and disabled persons.
-
Ability to generate enthusiasm and cooperation from others.

Ability to develop and maintain good interpersonal relationships.
-
A genuine interest in and respect for people and a desire to be of service to them.

Knowledge of Federal and State regulations and reimbursement systems..
DUTIES AND RESPONSIBILITIES:
-
Attends clinical meeting daily when scheduled to work, at a minimum, to ensure systems are
-
in place and to monitor sustainability to insure appropriate care is provided to all residents
-
Joins meeting preferably with both the Administrator and Director of Nurses involving
-
unusual occurrences, facility meetings and IDT as needed
-
Establishes rapport with residents, direct care personnel, administrative personnel of the
-
facility and Rockport Healthcare Services associates.
-
Provides education programs to address resident and regulatory noncompliance
-
Makes rounds and identifies physical environment, nursing practice, safety practices,
-
infection control practices and regulatory violations.
-
Reviews recommendations from other healthcare professionals (Medical Record, Registered
-
Dietitian, Pharmacist, Rehabilitation Staff, Social Service Consultants, etc.) to insure there is
-
a system in place for followed up.
-
Assesses the resident to determine the accuracy of documented assessments but not limited to
-
MDS, Change of Condition, and accidents or all other safety-related events.
-
Collaborates with the interdisciplinary team members and physicians to implement the
-
nursing process.
-
Writes a comprehensive report as needed and reports to the DON daily pertinent events,
-
changes, updates relating to the patient, patient care, family member and responsible parties
-
Informs supervisor immediately of urgent concerns and problems
-
Supports the facility during regulatory compliance visits from outside agencies
-
Incorporates the review of facility policies and procedures in the evaluation of systems
-
Assist the facility in implementing company policies and procedures along with the
-
corresponding company forms and provide necessary training/education.
-
Perform resident record review as deemed necessary and appropriate.
SUPERVISION
-
Participates in the review of performance for the Director of Nursing Services and makes
-
recommendations to the Administrator.

Assists in investigation of unusual occurrence or alleged abuse and promptly notify the
-
Director of Nursing Services and Administrator of finding/recommendations.
-
Reviews and assists the Director of Nursing Services with the correction of findings and
-
deficiencies of Department of Public Health.
-
Makes staffing recommendations to the Director of Nursing Services to ensure adequate 24-
-
hour nursing coverage of the facility each day.

Oversight of Quality Measures or other relevant clinical/quality metrics and offer the
-
guidance to continually improve the facility quality indicators
