Visit Nurse-RN

Perform recertification's, resumptions of care, discharge assessments, and routine client visits.
Counsel clients and families about nursing and related needs.
Communicate with the Nurse Case Manager about healthcare provider orders, necessary equipment, incident reports, and changes in client condition.

Coordinates the planning for delivery of nursing care with the Director of Nursing and administers appropriate clinical decisions in the delivery of patient care. Proactively updates client/patients’ POC, health changes, and other related incidents. Demonstrates knowledge of age-specific differences in the patient population including physiological and developmental differences unique to each group. Ensures safe care to patients; adheres to all policies, procedures, and standards, including time management, supply management productivity and quality of service Willingness to travel within assigned geographic region. Flexibility to work alternate shifts on short notice and can be on call for field visits as determined by business needs. Administers CPR and other emergency procedures, as necessary. Qualifications: Licensed as a Registered Nurse through the State Board of Nursing required. Home health experience preferred – Can perform assessments, wound care, medication management, infusions.
