Position Summary:
The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse or Physician. The LPN is responsible for delivering safe, compassionate, and effective nursing care while supporting residents’ well-being and maintaining compliance with regulatory standards.
Key Responsibilities:
· Provide direct nursing care to residents according to care plans and physicians’ orders.
· Administer medications and treatments accurately and in a timely manner.
· Monitor and document residents’ health status, reporting any significant changes to the RN or physician.
· Assist with resident assessments and the development of individualized care plans.
· Supervise and direct CNAs and other nursing assistants in delivering daily care.
· Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations.
· Respond to emergencies and follow proper protocols.
· Support residents and families with health education and emotional support.
· Maintain compliance with infection control, safety, and HIPAA standards.
· Participate in interdisciplinary team meetings, audits, and in-service training.
Qualifications:
· Valid LPN license in the state of employment
· Graduate of an accredited Licensed Practical Nursing program
· CPR/BLS certification (required or able to obtain upon hire)
· At least 1 year of nursing experience (long-term care preferred, but not required)
· Ability to communicate effectively and compassionately with residents, families, and team members
· Familiarity with electronic medical records (EMR) systems preferred
· Strong critical thinking, organizational, and time-management skills
Physical Requirements:
· Ability to lift/move up to 50 lbs
· Frequent standing, walking, bending, and reaching
· Ability to respond quickly to resident needs or emergencies
