$37.50 - $56.25EDUCATION/EXPERIENCE:
- Graduation from an accredited School of Nursing with two years' experience as an RN required. BSN preferred.
- Palliative care experience preferred. Certified Hospice and Palliative Care Nurse Certification preferred.
- Bilingual preference with fluency in Spanish preferred.
LICENSURE, CERTIFICATION, and/or REGISTRATION:
- Current license as a Registered Nurse in the State of applicable state required.
- BLS required.
ESSENTIAL FUNCTIONS:
- Ensures quality patient care by adhering to the policies of WFBMC, involving patients and their families in identifying their needs, creating a plan to meet those needs, and effectively communicating the plan across the continuum of care. Identifies barriers to efficient and effective management of patient care and seeks strategies to remove them.
- Works collaboratively with the ED, Care Coordination, Inpatient teams, Palliative Care, and Community Resources to ensure that patients receive the right care at the right time and in the right place. Assesses patient/family needs and facilitates implementation of the appropriate level of engagement.
- Implements outreach efforts to establish and maintain positive working relationships with key hospital team members and community agencies.
- Manages Palliative Care Consult requests by triaging referrals, assesses and facilitates the appropriate level of engagement.
- Acts as a resource for staff, patients and family members. Develops and leads palliative care education efforts throughout the medical center.
- Demonstrates ability to function in a professional practice model through active participation in shared governance and acceptance of personal responsibility for professional growth.
- Demonstrates responsibility for personal development by participating in continuing education offerings. Seeks new opportunities for self- improvement. Seeks and applies constructive feedback to improve performance.
- Provides age/developmentally appropriate patient care in accordance with Age-Specific guidelines for the specific age groups served.
- Attends team meetings to increase knowledge base including case conference meetings and team development.
- Manages outpatient Palliative Care patient needs. Facilitates scheduling for the Palliative Care Oncology Clinic and Cardiology Clinic-VAD evaluations.
- Serves as a clinical resource to physicians receiving transfer requests through the PAL line.
- Attends biweekly Quality Collaborative with Care Coordination as a subject matter expert to identify proactively, priority patients for Palliative Care Consults.
- Collects and maintains up to date patient disposition data. Delivers reports that demonstrate the Palliative Care Programs impact on mortality and readmissions.

- Excellent oral and written communication skills

- Demonstrates ability to work in fast paced environment with multiple interruptions
- Expertise in MS Office and ability to prepare program reports efficiently and frequently
- Ability to create and deliver presentations is essential
