Job Description Summary:

The Licensed Vocational Nurse (LVN) provides professional nursing care for patients following established standards and practices. Provides supervisory functions for nursing staff, and is the clinic’s only advice nurse.

Essential Functions and Responsibilities: 

Supervision:

  • Assists in the recruitment, management, and retention of clinical staff.
  • Responsible for the day-to-day oversight of clinical staff.
  • Orients new clinical staff to the practice.
  • Participates in the evaluation process of clinical department staff.
  • Updates clinical policies and procedures annually.
  • Responsible for reporting risk management issues to management.
  • Strives to attain individual goals negotiated with administrators and physicians.
  • Assumes responsibility for own career development, CPR certification, and licensure requirements.

Assessment:

  • Performs thorough assessment of patients, both in person and by telephone.     Documents pertinent information in the medical record.

Planning:

  • Participates in establishing and updating standards of care for patients.  Utilizes interdisciplinary collaboration in planning care.
  • Participates in the development and implementation of nursing policies, standing orders, and patient education materials.
  • Review upcoming schedules in advance, ensuring timely and accurate preparation for procedures, and ensures that necessary medications and supplies are on hand as needed.

Implementation:

  • Prepares and administers physician-ordered chemotherapy and other medications according to established policies and procedures.
  • Performs physician-ordered treatments as per protocol.
  • Responds in an appropriate and timely manner to anticipated or unexpected clinical problems.

Patient Evaluation

  • Participates and contributes to ensure quality patient care.
  • Revises procedures according to patient needs, new information and technology.
  • Documents evaluations in medical record.
  • Answers all patient calls and returns all patient messages by the end of each day.

Patient Education: Patient and Families

  • Provides patients and their families with verbal and written information regarding their illness, therapies and general healthcare.
  • Documents teaching activities and patient progress in medical records. 

Communication:

  • Ensures a professional and helpful working environment by upholding the standards as adopted by the practice.
  • Demonstrates ability to listen and establishes an environment that enables employees, patients and families to express their feelings.
  • Demonstrates accepting and unbiased behavior toward employees and patients.
  • Recognizes self-limitations and seeks appropriate assistance when necessary.
  • Recognizes and resolves conflicts with assistance from administrator if necessary.
  • Recognizes and supports fellow staff members under stress.
  • Participates in staff meetings and other professional activities.
  •  Demonstrates ability to listen to peers and supervisors in order to absorb and understand clinical and administrative operations and protocols.
  • Demonstrates resourcefulness in obtaining answers when presented with problems from both clinical and administrative personnel.

Administration:

  • Participates in maintaining equipment and supplies. Orders and tracks all equipment necessary to meet expected patient needs – drugs, solutions, and external dressings and devices.
  • Ensures that encounter forms are complete and accurate.
  • Keeps treatment rooms and lab clean, orderly and stocked. Tracks expiration dates and returns goods for refund as necessary.
  • Follows all safety rules and regulations as set by the medical practice and OSHA.
  • Demonstrates knowledge and competence in use of all medical equipment.
  • Processes disability and insurance forms.  Flags appropriate records to be copied for release of records to patients, other physicians and/or facilities.

Advocacy:

  • With other healthcare providers, ensure that the patient and family possess knowledge and understanding of the patient’s illness and therapy in order to attain self-management, participation in therapy, and optimal quality of life.

Qualifications:

Education:

  • Graduate of an accredited school of nursing with a degree in nursing, with a minimum of two years’ experience in nursing (clinical understanding required, willing to teach Urology specifics).

License & Certifications:

  • Current California license as a LVN.
  • BLS Certification (American Heart Association) required.

Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. QUALIFICATIONS • Graduate of an accredited school of professional nursing. • Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing. • Possess one or more year’s experience in a skilled nursing care facility. • Ability to read, write and speak the English language. • Possess mental and physical health. GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.
