- Provide the delivery of culturally sensitive team-based care models in delivering expert nursing care to a diverse adult patient population who require anticoagulation therapy for the Anticoagulation Services at Domino?s Farms.
- Perform telephone triage, including assessment, screening, and clinical advice; patient education, documentation with follow-up, coordination with ancillary services and related departments, and other duties as assigned.
- Participate as a member of the collaborative health care team, working closely with patients and their families.
- Provide functional supervision to assistive personnel; collaborate with colleagues, other disciplines, and health care/community services to promote continuity of care for optimal patient outcomes.
- Accurate and efficient documentation of nursing care. Independently and completely performs focused assessment to provide best practice patient care for patients and families as appropriate.

Required Qualifications*
- Current licensure to practice nursing in the State of Michigan (RN)
- Minimum of three (3) years RN experience in the past five (5) years with adult medicine patients on a medical/surgical inpatient unit or in a Cardiovascular Medicine Ambulatory Care Clinic
- Minimum of three (3) years out of the last five (5) years of RN experience providing care to adult patients whose primary diagnosis is cardiovascular in nature, including patients with diagnosis of heart disease, atrial fibrillation, valve replacements, deep vein thrombosis and stroke.
Desired Qualifications*
- Bachelor of Science in Nursing desired.
- Proficiency with MiChart in-basket management or documentation of patient encounters in electronic health record, e.g., Epic as indicated by interviews, references, and/or performance evaluation in the past two (2) out of five (5) years RN experience
- Experience providing patient/family education in past two (2) out of 5 years of RN experience.
- Demonstrated excellent verbal and written communication skills as evidenced by interviews, evaluations, and references in past two (2) out of 5 years of RN experience.
- Experience in assessing, treating, and evaluating patient's response to IV and oral anticoagulation therapy.
- Previous experience using clinical protocols to make decisions for adult patients.
- Previous experience in an anticoagulation clinic
- Hours/Week: 32 hours/week.
- Shift/Hours/Days: Day Shift; Mon-Fri, 8-hour shifts, with possibility for evenings and Saturdays may be required (nontraditional including further weekends/holidays commitment). Hours may be variable based on patient and volume needs, with flexibility required.
