POSITION DESCRIPTION:

The incumbent for the Senior Certified Nursing Assistant (SCNA) position is responsible to provide direct nursing care to residents of the State Veterans’ Nursing Home of Florida, under the supervision of a licensed nurse, in accordance with facility policies and procedures. Incumbent will assist with all aspects of resident care as assigned by the shift supervisor, including activities of daily living, ambulation, and mobility in facility. Respond promptly and courteously when answering call lights. Observe, identify, and report resident concerns or needs to the shift supervisor. Complete assignments timely, completely, accurately and in a cooperative manner. Follow established performance standards and perform duties according to nursing service and FDVA policies and procedures.

Provide the necessary assistance when giving residents the following activities of daily living nursing care: oral hygiene, bathing, dressing, and grooming, hair, and nail care, shaving, toileting, eating and drinking, and mobility
Assist as needed with related functions to, for, and with residents to include activities such as escorting to meals, assisting with laundry, and assisting with participation in activities
Perform restorative measures such as obtaining and recording vital signs/weights, and assistance with self-administration of medications. Document services provided within facility software/databases
Consistently work cooperatively and respectfully with residents, family or resident representatives, facility staff, physicians, ARNP’s, consultants and ancillary service providers
Follow Residents Rights policies at all times
Maintain confidentiality of resident information
Assist with monitoring and accountability of residents to enable medical personnel to accomplish health related tasks
Attend all in-service programs, training classes, and meetings
Assume responsibility and accountability for information contained in the Employee Handbook
Observe all facility safety, infection control and quality improvement, HIPAA policies and procedures, and internal quality improvement processes
Performs other related duties as required

Routinely observe, identify, and report resident concerns/needs to supervisor. Provide escort to resident appointments when required. Complete assignments timely, completely, and accurately. Accept assigned duties in a cooperative manner. Perform restorative nursing measures as outlined in the care plan. Perform simple nursing measures such as, but not limited to, applying splints, and positioning devices, making occupied and unoccupied beds, changing linens, etc. Measure and record vital signs as assigned. Measure and record intake and output according to the care plan. Observe, monitor, and report symptoms of potential skin breakdown and/or pressure ulcers to primary nurse. Use appropriate technique for mechanical lifts, transferring, turning, and positioning, range of motion exercises recognizing changes that place a resident at risk for pressure ulcers and falls. Provide appropriate end of life care and use appropriate techniques for assisting and Provide the required nursing care for assigned residents in Activities of Daily Living using proper technique to include mouth care, bathing, dressing, grooming, hair and nail care; shaving, toileting; feeding/assistance with eating; maintaining hydration; and mobility as outlined in the care plan. Follow facility established policies and procedures when providing all care and dining room protocol. Responding to the cognitively impaired resident or the resident with difficult behaviors including Dementia/Alzheimer’s. Observe conditions and report behavioral changes to primary nurse/RN Supervisor. Ensure that all resident safety devices are used as outlined in the care plan. Provide escort for resident appointments etc. as assigned. Document all care given on the appropriate form and/or utilizing the electronic patient care documentation system. Maintain confidentiality of residents. Ensure efficient use of supplies and document in the electronic inventory tracking system. Respond to resident’s requests and needs timely and appropriately. Answer resident call lights promptly. Notify nurse of any changes in resident’s condition. Complete assignments from the primary nurse or RN supervisor within the time allowed. Attend all required facility training. Perform other related work as required. This position is in the Career Service system.
