Job Duties
- Coordinates assignments and ensures the unit is prepared and runs efficiently at each session. Prepares exam rooms, anticipating additional supplies as indicated. Is attentive to the environment of care and maintains a neat and orderly work area. As part of the team, coordinates patient scheduling of tests, treatments, disease management, case management, pre-certifying patients for procedures, processes referrals and follow up of appointments, tests and plan of care per the provider’s instructions. Maintains open communication with providers regarding incomplete orders, patient tests / consults. Documents care and follow up in the medical record.
- Coordinates ordering supplies, medications etc pertaining to clinical area to promote smooth and efficient operations.
- Documents the appropriate assessment intervention plan and evaluation to reflect continuity of visits in a clear and concise manner. Documents and updates the medical record as required including but not limited to, assessment forms within 3 visits, allergies, summary list, medication list, immunization list, teaching form etc.
- Participates in Quality Improvement processes and identifies and implements potential corrective actions. Submits monthly audits timely and accurately.
- Performs all other duties as assigned
- Performs interventions in accordance with current policy and procedures, safety guidelines and infection control principles. Communicates concerns to a supervisor /physician and/ or the appropriate manager i.e. director of safety, infection control etc.
- Prepares patients to be seen by the provider. Gathers age specific information, performs age specific technical aspects of patient care (i.e., vital signs, head circumference, vision, hearing screenings, injection, dressing change) according to established professional guidelines and communicates to MD or team as appropriate. Chaperones patients as indicated. Administers ordered treatment & medication safely, according to policy and documents same.
- Promotes the professional image of nursing through community activities (hospital committees, civic organizations, parish nursing, volunteerism, etc.)
- Works with team and evaluates walk-in patients or takes calls from patients to assure the best outcome for the patient. Demonstrates professionalism and sound judgment in practice, crisis or emergency situations and seeks assistance as needed. Transmits pertinent information to the appropriate person (provider, supervisor etc) in a timely manner and documents same. Demonstrates excellent communication skills ie phone, with patients, visitors, staff and providers etc.
- Patient-Centered Medical Home (PCMH) Prepares for and participates in team meetings and huddles. Collaborates in developing team priorities and patient goals and care plans. Active in patient education, goal setting, self-management teaching and coaching. Medication reconciliation and education. Chronic disease care management.
- Current NJ RN license
- Graduate of an accredited NLN Nursing program
- Recent/Current Nursing experience required
- Current BLS Heart Saver/Health Care Provider
- Experience in ambulatory/outpatient services preferred.
- Ability to manage and provide safe nursing care in a high volume practice and demonstrates a positive, professional manner.
